To request for patient medical records, please email a signed HIPAA release form by the patient to: upstateurgentcarestaff@gmail.com
We have a central medical records office hence we are only able to respond to the requests sent by email to: upstateurgentcarestaff@gmail.com
If requests are sent by any other means, they may not not be honored in time as it may follow a chain to get through to medical records department.

Urgent Care conveniently located across from Easley Post Office at 807 S Pendleton Street, Easley, SC, 29640. We are open Monday to Friday 8am-7pm & Saturday to Sunday 8am-6pm. We provide immediate medical care to all ages including children. You can walk-in or make an appointment at www.upstateurgentcare.net, Some of the services we provide includes physical exam, x-ray and lab services, STD testing as well as cough, cold & flu treatment. Our staff also provides DOT physicals, pre-employment drug screens, and Workers Comp. Injury treatments. We look forward to getting you better....faster!!!
